The guideline covers the care provided by primary
and secondary healthcare professionals who have direct contact with, and make
decisions concerning the care of, patients with heart failure. The guideline
also addresses the interface between primary and secondary care, including in
what circumstances patients should be referred to or admitted to secondary care.
